Melasma: The Device Combination Protocol That Avoids Rebound
Melasma punishes aggressive treatment. Devices still have a role — but only inside a conservative protocol layered on top of photoprotection and barrier care. This is the combination order that manages relapse instead of triggering it.
Prerequisites Before Any Device
- SPF 50+ daily compliance for 4+ weeks (no exceptions)
- Barrier repaired — no active stinging or desquamation
- Physician-managed topicals/orals established where applicable
The Conservative Combination Order
- Low-fluence large-spot 1064 nm: \”toning\” passes, 2-4 week intervals, endpoint = mild or no visible change
- Low-fluence picosecond: photoacoustic pigment fragmentation with minimal heat for stubborn epidermal component
- Narrowband DPL: addresses the vascular component that feeds melanocyte reactivity
The Rebound Tripwires
Visible darkening after any session: stop devices, return to foundation care 4-8 weeks. High-fluence \”spot blasting\”: never — it is the single most common cause of post-laser melasma worsening. Combine only one device escalation at a time, reassessing at 4-week intervals.
Equipment fit: picosecond platforms for low-heat pigment work, DPL + 1064 combos for the vascular-plus-toning stack. Client communication script lives in the photofacial plan — same checkpoint discipline applies.
