Beyond the Fitzpatrick Chart: Reading Skin Type in Real Consultations

The Fitzpatrick scale is a starting frame, not an answer. Real consultation assessment blends the chart with tanning history, ethnic background, current skin state and — critically — what the client\’s skin has actually done under sun. Two type-III clients can need opposite settings.

The Practical Assessment Stack

  1. Chart baseline: hair/eye color and burn-tan answers give the opening estimate (I-VI)
  2. History override: \”Do you tan or burn?\” beats the chart — a self-reported easy tanner with type-III coloring behaves as IV
  3. Current state check: active tan, recent sun exposure, post-inflammatory marks from old acne — each forces settings down regardless of baseline
  4. Ethnic pigmentation pattern awareness: some backgrounds carry higher PIH reactivity than their Fitzpatrick number implies — treat the reactivity, not the number

Where the Assessment Changes Decisions

Document It

Record the four-step assessment, not just the number: \”III by chart, tans easily, active shoulder tan, PIH history on chin\” tells the next operator everything the number alone hides. The PIH risk frame comes from the prevention protocol.
